Contribution of Professor Nadiya Boyko: Intestinal dysbiosis and chronic inflammation as hidden triggers of diseases of civilization
The chapter, written by Professor Nadia Boyko in collaboration with Professor Godfrey Grech and Dr. Moien Khan, reveals detailed molecular and immunological mechanisms of how intestinal imbalance becomes a trigger for the development of most known chronic pathologies.
1. The phenomenon of subclinical low-grade inflammation (Low-Grade Inflammation)
Traditional medicine is accustomed to diagnosing inflammation when it is clearly expressed (for example, acute colitis, pancreatitis or infection). However, science proves that the most dangerous is long-term, hidden, asymptomatic chronic inflammation of low intensity . Its initiator is almost always a violation of the permeability of the intestinal barrier, known as the "leaky gut" syndrome . The intestinal wall is covered with a thin single-layer epithelium and a protective gel of mucin, which normally does not allow large molecules and bacterial components to enter the internal environment of the body.
In dysbiosis (reduction in the diversity of beneficial bacteria and excessive growth of opportunistic microbes), the protective biofilm is destroyed. Bacterial endotoxins — primarily lipopolysaccharides (LPS) , which are components of the cell walls of gram-negative bacteria — begin to massively penetrate through the epithelium into the bloodstream. This process, called metabolic endotoxemia, activates immune cells in the blood, causing them to constantly secrete pro-inflammatory cytokines (interleukin-1β (IL-1β), IL-6, tumor necrosis factor alpha (TNF-α)). The body finds itself in a state of permanent systemic inflammation, which gradually destroys blood vessels, joints, nerve cells, and tissues of internal organs.

2. The microbiome chain: from the gut to the heart, brain and blood vessels
The section analyzes in detail the cause-and-effect relationships between the state of the microbiota and specific nosologies:
Metabolic syndrome and obesity : An imbalance between the Firmicutes and Bacteroidetes bacteria dramatically alters energy metabolism. Certain microorganisms are able to break down complex carbohydrates too efficiently, leading to excessive calorie absorption even with a moderate diet.
Type 2 diabetes : Loss of specific protective bacteria (e.g., butyrate-producing bacteria and mucin-degrading Akkermansia muciniphila ) disrupts insulin receptor signaling pathways, provoking insulin resistance.
Cardiovascular risks and atherosclerosis : Professor Boyko's research has shown a clear correlation between the presence of certain transient bacteria in the intestine (for example, Proteus vulgaris in high concentrations) and a sharp increase in cholesterol and uric acid levels and a drop in secretory immunoglobulin A (sIgA) in the intestine, which accelerates vascular calcification and sclerotization.
Neurodegenerative diseases and depression : Through the Gut-Brain Axis, bacteria directly modulate the nervous system. Dysbiosis blocks the synthesis of serotonin and GABA, which exacerbates anxiety, panic attacks, Parkinson's disease, and dementia.
3. Revolutionary methods of diagnosis and therapy
The authors of the section emphasize that treating these conditions with random probiotics “blindly” is ineffective and dangerous. Science requires a precise definition of the etiology of dysbiosis.
Instead of outdated cultures for dysbiosis, the book describes the use of next-generation sequencing (NGS) 16S rRNA technologies and metagenomic analysis of bacterial DNA, which allows for the detection of species diversity (Shannon index), enterotype, and metabolic potential of microflora.
